In the realm of radiation therapy, where precision is paramount and safety is non-negotiable, Laser QA (Quality Assurance) is an important aspect. Therefore, routine checks of lasers of linear accelerators (linacs) and CT simulators is crucial.

Understanding Laser QA

Laser QA is performed daily by the radiation therapists that ensures the lasers used in radiation therapy intersect within a specific tolerance at the isocenter. The isocenter is the focal point where the radiation beams converge and the gantry, collimator and couch all rotate around. We also use these lasers to triangulate and align patients for treatment setup. This precision is vital to the success of the therapy, as any deviation could lead to unintended consequences, compromising both the efficacy of treatment and patient safety.

The Laser QA Guide

Performing Laser QA involves a step-by-step process that radiation therapists follow diligently as part of their daily quality checks. Each department may have their own policy and procedure on how to perform this specific test. There are a variety of methods and tools to use to perform this check. Since this test is a part of the daily checks performed by the radiation therapists, they are often the first line of defense in ensuring patient safety. Therefore when performing this test, a keen attention to detail is required.

  • Daily Check Routine:
    • Laser QA is integrated into the daily check routine for linacs and CT simulators.
    • Radiation therapists typically undertake this responsibility during morning QA tests.
  • Isocenter Precision:
    • The lasers must intersect at the isocenter, the focal point of the treatment.
    • Each machine type has specific tolerances. Any deviation is meticulously noted and addressed promptly to maintain precision.
  • Tolerance Variations:
    • Tolerance levels in Laser QA vary based on the machine type and the treatments it facilitates.
    • Machines dedicated to Stereotactic Radiosurgery (SRS) and Stereotactic Body Radiation Therapy (SBRT) uphold stricter tolerance compared to those focused on non-Intensity Modulated Radiation Therapy (IMRT) and IMRT treatments only.

 

The Role of Tolerance in Laser QA

An important aspect of Laser QA is understanding tolerance. Tolerance refers to the permissible deviation from the ideal isocenter alignment. In the world of radiation therapy, precision is the name of the game, and tolerance sets the rules.

  • Machine Type Matters:
    • Different machines have different tolerances based on their intended use.
    • SRS and SBRT machines, dealing with highly precise treatments, operate within narrower tolerance margins because there is a demand for a higher level of accuracy.
    • Machines catering to 2D or 3D (aka non-IMRT) treatments have a different tolerance than those that have the ability to deliver IMRT treatments.
    • Laser QA acts as a vigilant gatekeeper, adhering to the specific tolerance requirements of each machine to guarantee precision in treatment delivery.
